I, the undersigned, am the parent or legal guardian of the minor named for this registration. I hereby grant Victory Baptist Church permission to photograph, video record, and/or otherwise capture the likeness of my child. I authorize the use of these images and recordings for lawful purposes, including but not limited to publicity, marketing, educational materials, and social media, without payment or other consideration.
I waive any right to inspect or approve the finished product or the use to which it may be applied. I release and hold harmless Victory Baptist Church, its representatives, and employees from any claims, demands, or causes of action arising from the use of these images or recordings.
